These forms will provide us with the pertinent information related to your medical history, which will allow for a more thorough and efficient physical examination. Each of the following forms and documents must be completed and returned to the Southeastern Louisiana University Sports Medicine Department by all student-athletes. If you have sustained an injury prior to arriving at Southeastern Louisiana University, you may need additional diagnostics tests before allowed to participate in a sport. The athlete and/or parent are financially responsible for any diagnostic testing.
